ABSTRACT

@#Abstract: Objective To investigate the diagnostic value of joint detection of Mycobacterium tuberculosis rifampicin resistance gene (Xpert MTB/RIF), Mycobacterium tuberculosis ribonucleic acid (TB-RNA) and Mycobacterium tuberculosis deoxyribonucleic acid (TB-DNA) in bronchoalveolar lavage fluid for smear-negative pulmonary tuberculosis. Methods A total of 806 patients with suspected smear-negative pulmonary tuberculosis admitted to our hospital from May 2020 to July 2022 were selected, 506 patients diagnosed as bacterial negative pulmonary tuberculosis by clinical, X-ray and sputum samples were classified as bacterial negative pulmonary tuberculosis group, and the other 300 patients with non-tuberculous pulmonary disease were classified as non-tuberculous pulmonary disease group. XpertMTB/RIF, TB-RNA and TB-DNA in bronchoalveolar lavage fluid of all patients were detected. With clinical, X-ray and sputum specimen examination of mycobacterium tuberculosis as the gold standard, the diagnostic efficacy of alveolar lavage solution Xpert MTB/RIF, TB-RNA and TB-DNA alone and in combination was analyzed. Results The positive detection rates of Xpert MTB/RIF, TB-RNA and TB-DNA in bronchoalveolar lavage fluid of the smear-negative pulmonary tuberculosis group and the non-tuberculosis pulmonary disease group were 69.96% (354/506) and 2.67% (8/300), 61.46% (311/506) and 5.00% (15/300), and 63.64% (322/506) and 8.00% (24/300), respectively. The rates in the smear-negative pulmonary tuberculosis group were higher than those in the non-tuberculosis lung disease group, and the differences were statistically significant (χ2=342.005, 246.930, 235.687, P<0.01). Compared with the gold standard, the sensitivity, specificity, accuracy, positive predictive value and negative predictive value of Xpert MTB/RIF in the diagnosis of smear-negative pulmonary tuberculosis were 69.96%, 97.33%, 80.15%, 97.79% and 65.77%, respectively; those values of TB-RNA were 61.46%, 95.00%, 73.95%, 95.40% and 59.38%, respectively; those values of TB-DNA were 63.64%, 92.00%, 74.19%, 93.06% and 60.00%, respectively; those values of combined diagnosis with Xpert MTB/RIF, TB-RNA and TB-DNA were 61.26%, 100.00%, 75.68%, 100.00% and 60.48%, respectively; the specificity and positive predictive value of combined detection were higher than those of single detection (P<0.05). Conclusions The joint detection of Xpert MTB/RIF, TB-RNA and TB-DNA in bronchoalveolar lavage fluid can improve the diagnostic efficacy of smear-negative pulmonary tuberculosis and is worthy of clinical promotion and application.

ABSTRACT

OBJECTIVES: This study aimed to determine the concordance between CT and nucleic acid testing in diagnosing coronavirus disease (COVID-19) outside its district of origin (Wuhan, China). METHODS: Twenty-three consecutive patients with COVID-19, confirmed by nucleic acid testing, were enrolled from two designated hospitals outside the district of disease origin. We collected clinical, laboratory, and CT data and assessed the concordance between CT manifestations and nucleic acid test results by comparing the percentage of patients with and without abnormal CT findings. Furthermore, using Chi-square tests, we analyzed the differences in CT manifestations between patients with and without an exposure history or symptoms. RESULTS: Multiple ground-glass opacities (GGOs), with or without consolidation, were observed on the initial CT scans of 19 patients (82.6%), whereas the remaining 4 (17.4%) showed no CT abnormalities, indicating that the initial chest CT findings were not entirely concordant with the nucleic acid test results in diagnosing COVID-19. Among the latter 4 patients, we observed multiple GGOs with and without consolidation in 2 patients on the follow-up chest CT scans taken on days 7 and 14 after admission, respectively. The remaining 2 patients showed no abnormalities on the follow-up CT scans. Furthermore, abnormal CT findings were found more frequently in patients who had been exposed to COVID-19 in its district of origin than in those who had not been exposed and in symptomatic patients than in asymptomatic patients (all p<0.05). CONCLUSIONS: Patients with positive results on nucleic acid testing may or may not have the abnormal CT manifestations that are frequently found in symptomatic patients with a history of exposure to the district of COVID-19 origin.

ABSTRACT

@#Acute pulmonary embolism (PE) is a common disorder with significant morbidity and mortality in patients who underwent pulmonary ground-glass nodules (GGN) resection. We should make efforts to increase surgeons' awareness of risk factors of PE and their understanding of the effectiveness of prevention strategies. Using the optimal risk assessment model to identify high-risk patients and give them the individualized prophylaxis. Early diagnosis and accurate risk stratification is mandatory to reduce the rates of PE, to decrease health care costs and shorten the length of stay. This article summarizes the risk factors, diagnostic process, risk assessment models, prophylaxis and therapy for the PE patients who underwent GGN resection.

ABSTRACT

Abstract Achillinoside was isolated from methanol extract of Achillea alpina L., Asteraceae. The structure of the compound was characterized based on various spectrum data, including IR, HR-ESI-MS, 1D and 2D NMR. The cardiovascular protective effect of achillinoside was tested on H2O2-induced H9c2 cells. In our research, achillinoside could increase the cell viability dose-dependently in H2O2-induced H9c2 cells. In addition, the levels of caspase-3/9 cells were significantly decreased in H2O2 and achillinoside incubated H9c2 cells.

@#Objective    To investigate the correlation between lung ultrasonography and pulmonary complications after cardiac surgery. Methods    Fifty-two patients after cardiac surgery in our hospital from January to May 2017 were recruited. There were 27 males and 25 females, aged 60.50±10.43 years. Lung ultrasonography was performed by specially trained observers, video data were saved, and lung ultrasound score (LUS) were recorded. The correlation between the LUS and the patients' pulmonary function was evaluated. Results    LUS was 17.80±3.87, which was negatively correlated to the ratio of arterial PO2 to the inspired oxygen fraction (PaO2/FiO2) during examination, without significant difference (r=–0.363, P=0.095), but significantly negatively correlated to PaO2/FiO2 changes 24 hours postoperatively (r=–0.464, P=0.034). Conclusion    The changes of lung ventilation area may occur earlier than the changes of lung function. Bedside LUS is an effective method for clinical monitoring of pulmonary complications.

ABSTRACT

Abstract Vancomycin is a first-line drug for treating methicillin-resistant Staphylococcus aureus. Thrombocytopenia is a rare adverse reaction to vancomycin treatment, and there are no reports of vancomycin-induced thrombocytopenia (VIT) in infants. We describe the case of a 3-month-old girl who was diagnosed with purulent meningitis. After 13 days of treatment with vancomycin, her platelet count reduced to 8 × 109/L. Vancomycin was discontinued, and intravenous methylprednisolone was administered. The platelet count returned to normal after 4 days. Patients, especially young children, receiving vancomycin for a long clinical course should undergo careful monitoring of laboratory indicators and blood tests.

ABSTRACT

This study aimed to determine the impact of dentinal tubule orientation on dentin bond strength to provide a reference for clinical cavity preparation in resin-bonded restoration.Patients aged 13-16 years were selected,including 18 males and 21 females.Forty-eight human maxillary first premolars from orthodontic extractions were chosen to prepare the test models with the dentinal tubule orientations perpendicular and parallel to the bonding substrate.The test models in the vertical and parallel groups were divided into three groups:total-etching with 20% phosphoric acid,total-etching with 35% phosphoric acid and self-etching,with the dentinal tubule surfaces bonded with composite resin blocks in each group.After the standard test models of dentinal tubule-composite resin blocks were placed in distilled water and stored at 37℃ for 24 h,shearing tests were performed using a universal material testing machine at a crosshead speed of 0.5 mm/min.The bond strength values in the vertical group were 19.33±1.59 MPa for the 20% phosphoric acid group,21.39±2.34 MPa for the 35% phosphoric acid group,and 16.88±1.54 MPa for the self-etching group.The bond strength values in the parallel group were 24.53±1.99MPa for the 20% phosphoric acid group,25.16±2.88 MPa for the 35% phosphoric acid group,and 20.83±1.99 for the self-etching group.After using same total-etching adhesive,the shear bond strength of the parallel group was higher than that of the vertical group,and the difference was statistically significant (P<0.05).Regardless of vertical group or parallel group,the difference in the bond strength value between the total-etching groups and the self-etching group was statistically significant (P<0.05).It was concluded that the dentin bonding substrate which was parallel to the direction of the dentin tubule achieved an improved bond strength;the total-etching adhesives achieved higher bond strengths in dentin bond than the self-etching adhesives.

ABSTRACT

The effect of heart-type fatty acid binding protein [H-FABP] remains unclear on the quality of the prognoses of patients with acute coronary syndrome [ACS]. The authors searched the EMBASE, PubMed, Cochrane Library, and Medline electronic databases from their respective inceptions throughout June 2017. Odds ratios were pooled from individual studies, and conducted heterogeneity analysis, quality assessment, and publication bias analysis. A total of four studies with 1,994 patients were included [554 positive patients; 1,440 negative patients]. Pooled analysis of these studies showed that patients positive for H-FABP had higher incidence for mortality [OR, 6.39; p <0.01] and cardiac events [OR, 3.94; P<0.01] than negative patients, but myocardial infarction [MI] [OR, 1.43; P=0.32] showed no significant differences. ACS patients positive for H-FABP had higher rates in cardiac events and mortality. The incidence of MI showed no significant difference, possibly because this study was based on limited data from randomized studies. Additional substantial randomized controlled trials certainly needed to verify these discovery

ABSTRACT

Objective To analyze the medication regularity of TCM famous doctors for diabetes by using improved Apriori algorithm to obtain more efficient data mining methods. Methods This article put forward Apriori vertical data storage, and improved ADPM obtained by difference set method was used to conduct data mining to find out the medication regularity in Guo Jia Ji Ming Lao Zhong Yi Tang Niao Bing Yan An Liang Fang. Results After screening, 402 prescriptions were included, involving 24 kinds of high-frequency medicine, 15 high-frequency medical combinations, and 18 highly-dependent medical combinations, which were mainly tonifying deficiency medicine, clearing heat medicine, blood-activating and stasis-resolving medicine, and damp-draining diuretic medicine. Conclusion ADPM algorithm can be applied in the analysis on medication regularity and find out high-frequency medicine, medical combinations and medical dependent relation, with high efficiency.

ABSTRACT

We performed a meta-analysis of randomized controlled trials (RCTs) to investigate the efficacy and safety of ticagrelor (TIC) vs.clopidogrel (CLO) in patients undergoing percutaneous coronary intervention (PCI).In Jun 2016,a literature search was started and all the studies were conducted from 2010 to 2015.We systematically searched the literature through the MEDLINE database,Cochrane library,and EMBASE database.Quality assessments were evaluated with Jadad quality scale.Data were extracted considering the characteristics of efficacy and safety designs.Six RCTs enrolling 26 244 participants and satisfying the inclusion criteria were finally analyzed.There was a significant decrease of all-cause mortality (MD=0.83,95%CI=0.74-0.93,P=0.001) and myocardial infarction (MI) (MD=0.78,95%CI=0.70~.88,P=0.000).There were no significant differences in stroke (MD=1.34,95%CI=0.99-1.79,P=0.06),total bleeding (MD=0.97,95%CI=0.84-1.12,P=0.66),minor or major bleeding (MD=l.06,95%CI=0.94-1.19,P=0.35) in patients undergoing PCI after treatment with TIC vs.CLO.TIC could be more significant in decreasing all-cause mortality and MI than CLO,but there were no significant differences between TIC and CLO in inhibiting stroke,major bleeding,major or minor bleeding in patients undergoing PCI.

Objective@#To study the blocking effect and abrasion resistance of hydroxyapatite (HA) with different particle sizes and morphologyies on dentinal tubules from the qualitative and quantitative aspects. @*Methods @# Dentin discs were etched and divided into 4 groups randomly: 20 nm HA group; 30 nm HA group; 12 μm HA group; control group (without any experimental treatment). Each group was brushed with correspondent materials for 7 days. Then, the dentin disk was divided into two parts, the one was detected by the scanning electron microscopy (SEM); the other was observed by SEM after toothbrush abrasion test. The Image-Pro Plus 6.0 image analysis software was used to calculate the plugging rate of dentinal tubules. @*Results @#SEM showed that the blockage is granular, the plugging rate of the dentinal tubules were about 82% to 96%. 30 nm HA group (short rod) range the first in the average plugging rate, followed by the 20 nm HA group (needles) and 12 μm HA group (spherical). After mechanical brushing for 7 days, SEM images showed that deposited layer in each group was smooth and compact, and more than 63% of the tubules still had been blocked. @* Conclusion @#HA with different particle sizes and morphologies had good plugging effect and abrasion resistance on dentine tubules. The blocking effect of HA was affected not only by particle size but also by the morphology.

ABSTRACT

OBJECTIVE: To investigate the feasibility of using free gracilis muscle transfer along with the brachialis muscle branch of the musculocutaneous nerve to restore finger and thumb flexion in lower trunk brachial plexus injury according to an anatomical study and a case report. METHODS: Thirty formalin-fixed upper extremities from 15 adult cadavers were used in this study. The distance from the point at which the brachialis muscle branch of the musculocutaneous nerve originates to the midpoint of the humeral condylar was measured, as well as the length, diameter, course and branch type of the brachialis muscle branch of the musculocutaneous nerve. An 18-year-old male who sustained an injury to the left brachial plexus underwent free gracilis transfer using the brachialis muscle branch of the musculocutaneous nerve as the donor nerve to restore finger and thumb flexion. Elbow flexion power and hand grip strength were recorded according to British Medical Research Council standards. Postoperative measures of the total active motion of the fingers were obtained monthly. RESULTS: The mean length and diameter of the brachialis muscle branch of the musculocutaneous nerve were 52.66±6.45 and 1.39±0.09 mm, respectively, and three branching types were observed. For the patient, the first gracilis contraction occurred during the 4th month. A noticeable improvement was observed in digit flexion one year later; the muscle power was M4, and the total active motion of the fingers was 209°. CONCLUSIONS: Repairing injury to the lower trunk of the brachial plexus by transferring the brachialis muscle branch of the musculocutaneous nerve to the anterior branch of the obturator nerve using a tension-free direct suture is technically feasible, and the clinical outcome was satisfactory in a single surgical patient.

ABSTRACT

Qualitative and quantitative analyses of biological samples containing drugs, toxicants and endogenous substances play an important role in the researches of life sciences, as well as in new drug discovery and development. Biological samples are characterized by complex matrix, multiple endogenous interferences, significantly lower concentrations of measured analytes compared to endogenous components and small sampling volume. Consequently, it often requires bioanalysis methods with superior specificity, high sensitivity and good reproducibility. The two-dimensional liquid chromatography (2D-LC) technique, which allows for high peak capacity, significant reduced matrix effect and carryover of complex matrix samples and automated sample pre-treatment and analysis, has been the powerful solution to the separation and analysis of biological sample and widely applied to environment, food and pharmaceutical analysis. On the basis of introduction of the principle and equipments of 2D-LC, the application of this technique in the pharmacokinetics, toxicological and biological study was reviewed.

Objective To investigate the moderator of children' s perceptions quality of parental involvement on parenting style and children' s sport expectancy-value beliefs.Methods 595 students of grade 2 to 6 from five elementary schools were chosen and analyzed.Data were analyzed by SPSS16.0 software packet using independent sample t test,One-Way ANOVA,correlation analysis and hierarchical regression statistical analysis of regulation.Results Children' s expectancy beliefs was predicted by authoritarian and behavior encouragement(β=0.076,t =2.35,P=0.019 ;β =0.111,t =3.748,P =0.000).Children' s value beliefs was predicted by affect -warmed and behavior encouragement (β =0.070,t =2.883,P =0.004 ;β =0.070,t =2.745,P =0.006).Children' s sport behavior was predicted by trust-encouragement(β=0.294,t=3.193,P=0.001).The prediction of expectancy beliefs was significant positive by the interaction between indulgence and time involvement (β=0.078,t =2.065,P =0.039).The prediction of expectancy beliefs was significant negative by the interaction between authoritarian and time involvement(β=-0.084,t =-2.454,P=0.014).The prediction of expectancy beliefs was borderline significant positive by the interaction between neglecting and equipment provision(β=0.073,t =1.94,P =0.053).Conclusion Different parenting styles are significant predicators of children sport involvement.Children' s perceptions quality of parental involvement play an important role on parenting style and children' s sport expectancy-value beliefs.

<p><b>OBJECTIVE</b>To investigate the mutation in transcription factor paired box gene PAX9 in a mongolian family with non-syndromic oligodontia.</p><p><b>METHODS</b>Peripheral blood was collected from 17 core family members (9 unaffected, 8 affected) in this Mongolian family with non-syndromic oligodontia. Mutation in exons of PAX9 gene was identified by PCR amplification and DNA sequencing.</p><p><b>RESULTS</b>A point mutation c.87G > C at position 87 in exon 4 of PAX9 was identified from 8 affected members in the family, which were G/C heterozygous.While the 9 healthy members in the family were homozygous for C which was consistent with normal reference sequence in the GenBank(accession number: NC_000014).</p><p><b>CONCLUSIONS</b>The mutation of c.87G > C (p. Ala240Pro) in exon 4 of PAX9 was likely to cause the non-syndromic oligodontia in this Mongolian family.</p>

ABSTRACT

Hepatocellular carcinoma (HCC) is the fifth most common cancer in the world. Surgery offers the only hope for cure. However, the potentially curable method is only possible for a small proportion of those afflicted, for the rest, palliative treatment is indicated. Despite all the treatment options when used as monotherapy, patients with HCC have a poor long term prognosis. In this setting, multimodal and combination therapy has emerged as an alternative treatment modality for HCC. Studies have looked at various forms of combination therapy, including neoadjuvant/adjuvant/downstaging therapy for surgery and the combined modality of non-operative therapies. The novel molecular targeted therapies are also being used as combination regimens for surgery or other non-operative therapies. Some forms of combination therapies, including downstaging therapy for surgery, salvage transplantation, and molecular targeted therapy have been shown to provide survival benefits for well selected patients, and need to be encouraged in the future. And others such as pre-operative bridging therapy for liver transplantation, adjuvant therapy for hepatic resection and combination of local and regional therapies have also shown some benefits in preliminary results, which need confirmation in further studies. In conclusion, multimodal and combination therapy is an encouraging treatment modality for HCC. Future research should continue to unravel the role of combination therapy with properly selected patients and appropriate end points.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the efficacy and safety of thrombopoietin (TPO) on platelet engraftment in hematological malignancies patients after allogeneic haematopoietic stem cell transplantation (allo-HSCT).</p><p><b>METHODS</b>One hundred and twenty patients were enrolled in a multicenter, open-label, randomized, controlled clinical trial, and were randomized into 4 treatment groups following allo-HSCT. Group A was the control arm without TPO, while group B, C and D were trial arms with received 300 U×kg(-1)×d(-1) of TPO starting from day +1, +4 and +7, respectively. A total of 89 cases were evaluated, of which 22 cases in group A, 23 in group B, 20 in group C and 24 in group D. Efficacy evaluation (the time of platelet engraftment, the number of platelet transfusion) and safety evaluation \[adverse events, routine blood tests, liver and renal function, coagulation function and occurrence of graft-versus-host disease (GVHD)\] were observed.</p><p><b>RESULTS</b>The median platelet engraftment time in experimental groups (groups B, C and D) were on day (13.17 ± 2.89), day (12.15 ± 2.08), day (12.33 ± 1.76), respectively, and that in control group was on day (14.82 ± 5.05). There was statistically significant difference between two groups (P = 0.029), There were no statistically significant difference in the average amount of platelet transfusion, platelet engraftment time, and platelet nadir value among the 3 experimental groups. No significant adverse events were observed in experimental groups.</p><p><b>CONCLUSIONS</b>TPO administration following allo-HSCT for patients with hematologic malignancies appears to shorten platelet engraftment time. TPO given starting from day +7 is effective and safe.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the clinical features and treatment of placenta previa complicated with previous caesarean section.</p><p><b>METHODS</b>The clinical data of 29 patients with placenta previa complicated with a previous caesarean section (RCS group) admitted in Peking Union Medical College Hospital during a period from 2003 to 2011 were retrospectively reviewed and compared with those of 243 patients with placenta previa without a previous caesarean section (FCS group) during the same period.</p><p><b>RESULTS</b>There was no difference in the mean age (28.9±3.6 vs.28.1±4.5 years) and the average gravidity (2.35±1.48 vs.2.21±1.53) between RCS group and FCS group (all P>0.05).The RCS group had more preterm births (24.1% vs.13.2%), complete placenta previa (55.2% vs.4.9%), placenta accreta (34.5% vs.2.5%), more blood loss during caesarean section (1412±602 vs.648±265 mL), blood transfusion (51.7% vs.4.9%), disseminated intravascular coagulation (13.8% vs.2.1%), and obstetric hysterectomy (13.8% vs.0.8%) than the FCS group (all P<0.05).The preterm infant rate (30.0% vs.13.0%), neonatal asphyxia rate (10.0% vs.4.9%), and perinatal mortality rate (6.7% vs.0.4%) of the RCS group were higher than those of the FCS group (all P<0.05).</p><p><b>CONCLUSIONS</b>More patients had complete placenta previa and placenta accreta, postpartum hemorrhage, transfusion, uterine packing, obstetric hysterectomy, and perinatal morbidity in the placenta previa patients with previous caesarean section.The patient should be informed of the risk and unnecessary first cesarean sections should be avoided.</p>

ABSTRACT

Anti-bone resorption activity of pilose antler blood (Cervus nippon Temminck) were evaluated in ovariectomized Wistar rats. The rats were randomly divided into sham operated group (SHAM), ovariectomized group (OVX) and pilose antler blood treated group. The ovariectomized rats were treated with pilose antler blood orally in 4000l/kg daily doses for 10 weeks. Compared with SHAM group, serum 17 -estradiol level decreased significantly and osteocalcin level increased significantly in OVX group, indicating successful model of osteoporosis. The experiments showed that the bone mineral density of the lumbar spine and left femur in OVX group decreased remarkably compared to SHAM group but normalized by treatment with pilose antler blood. Additionally, serum levels of insulin-like growth factor-1and testosterone were lower obviously in OVX group than those in SHAM group but preserved by pilose antler blood treatment. However, no obvious changes in serum levels of calcium, phosphorus, total alkaline phosphatase and osteoprotegerin were observed among three groups. These results suggested that administration of pilose antler blood was effective in alleviating osteoporosis in ovariectomized rats.
